Publicó su primera novela, Estudio en escarlata, en 1887, iniciando una serie de relatos protagonizados por Holmes y el doctor Watson. También escribió novelas históricas, obras de teatro y ciencia ficción. Participó como médico en la guerra de los bóeres y defendió el espiritismo tras la muerte de su hijo en la Primera Guerra Mundial. Su legado literario sigue siendo influyente y ampliamente leído.
